Transition Programmes
Transition Students 14 plus years
All students aged from 14 years have an Individual Transition Plan (ITP). The ITP is a plan that is created with the students, parents, whanau, therapists and teachers to set goals so that the student can work towards their life aspirations. This is the beginning of the transition journey.
Transition Students 18-21 years
As students progress through their transition plan, a greater emphasis is placed on life, community, work readiness, and leisure skills that will enable each learner to up-skill for the life they want to live.
Transition students are based in a class setting relevant to their learning needs Current settings include; Transition @ Base and Transition @ Tētēkura (Phillipstown).
Transition Students (Final Year of School)
Students, parents, whanau, Enabling Good Lives, and Ferndale Staff, join together to form a transition team. The team come together to focus on the student, their life aspirations, and to map out a plan for the students life following school. The team work together supporting the student to transition throughout the final year. Each plan is individualised and is built around the students interests and strengths.
